These Terms & Conditions ("Terms") govern your use of the Quantify Specialty Care, LLC ("QSC," "Quantify," "we," "our," or "us") website at quantifyspecialtycare.com and any text-message (SMS) program you opt into with us. By using our website or by providing your mobile number to us in any enrollment, intake, consent, or contact form, you agree to these Terms.

1. Website use

The content on this site is provided for general informational purposes about QSC's services. Nothing on this site is medical advice, and nothing here creates a provider–patient relationship. For clinical questions, please contact your care team directly at 740-994-1811 or 1 (888) 536-9963.

You agree not to (a) attempt to gain unauthorized access to any portion of the site, (b) interfere with the operation of the site, or (c) use the site to transmit unlawful, harmful, or infringing content.

2. SMS / Text Messages

2.1 Program description

When you provide your mobile phone number on a QSC intake form, patient consent form, care-team contact form, or partner enrollment form — and check the box authorizing text messages, or otherwise opt in by replying to our messages — you are enrolling in Quantify Specialty Care's SMS/Text Messages program. Through this program, QSC may send you messages related to your care, including:

• appointment scheduling and reminders
• infusion, injection, or medication delivery coordination
• refill and prior-authorization updates
• care-team check-ins and clinical follow-up
• educational messages tied to your therapy
• billing or account notifications

2.2 Consent

By opting in, you expressly consent to receive recurring text messages from QSC at the mobile number you provided. Your consent is not required as a condition of purchasing any goods or services. Message and data rates may apply, depending on your mobile carrier and plan.

2.3 Message frequency

Message frequency varies based on how you communicate with your care team and the stage of your therapy. Active patients may receive more frequent messages around scheduling, infusions, and refills; other patients may receive fewer.

Supported carriers include AT&T, Verizon Wireless, T-Mobile, Sprint, Boost, Cricket, MetroPCS, U.S. Cellular, Google Fi, and most U.S. mobile carriers. Carriers are not liable for delayed or undelivered messages.

2.7 Privacy of text messages

Phone numbers and message content collected through the SMS program are used only to deliver the messaging services described above and to support your care. We do not sell, rent, or share your mobile information (including phone numbers and SMS opt-in data) with third parties or affiliates for marketing or promotional purposes. Vendors who help us operate the SMS program (for example, our SMS delivery platform) may receive your information solely to provide that service to QSC and are contractually restricted to that use.

To the extent text messages we send you contain Protected Health Information (PHI), they are governed by our HIPAA Notice of Privacy Practices, which is provided to patients at intake and is available on request from compliance@quantifysc.com. 2.8 Eligibility

The SMS program is intended for individuals located in the United States who are 18 years of age or older, or for the parent or legal guardian of a QSC patient who is a minor. By opting in, you represent that you meet this requirement and that the mobile number you provided belongs to you or that you have authority to receive messages at that number.

2.9 Changes to the SMS program

We may change or end the SMS program at any time, with or without notice. We may also change these Terms; the "Last updated" date above reflects the most recent revision.

3. Intellectual property

All content on this site — including text, graphics, logos, the Quantify wordmark, and the Quantify radial mark — is the property of Quantify Specialty Care, LLC or its licensors and is protected by U.S. and international intellectual-property law. You may not reproduce, distribute, or create derivative works from any portion of the site without our written permission, except for personal, non-commercial reference use.

4. Third-party links

This site may link to third-party websites or services that we do not control. We are not responsible for the content, accuracy, or privacy practices of any third-party site.

5. Disclaimers

The website and SMS program are provided on an "as is" and "as available" basis without warranties of any kind, express or implied, except as required by law. We do not warrant that the site or the SMS program will be uninterrupted, error-free, or free of viruses or other harmful components.

6. Limitation of liability

To the maximum extent permitted by law, QSC, its officers, directors, employees, contractors, and affiliates will not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the website or the SMS program — including but not limited to delayed or undelivered messages, carrier charges, or interruptions in service.

7. Governing law

These Terms are governed by the laws of the State of Florida, without regard to its conflict-of-laws principles. Any dispute will be brought exclusively in the state or federal courts located in Broward County, Florida.
